Appointment Policies

Below you’ll find a list of important clinic policies to be aware of in the preparation for, and during, your appointments. If you have questions about your appointment or concerns regarding any of the policies outlined below, please contact me directly via phone or email.

 

BOOKING PROCEDURES

Bookings can be made online, by phone, or by e-mail. For current up-to-date availability please click the “Book an Appointment” link in the navigation menu above. Please make sure to select the appropriate appointment type (Initial or Follow-up appointment) when booking online.

INTAKE FORMS

My practice is paperless. Please remember to fill out online intake and waiver forms before your initial appointment. If for any reason this is not possible, please contact me when booking your appointment.

CANCELLATION & MISSED APPOINTMENTS

Cancellation and rescheduling requires a minimum of 48hr notice, this is in order to allow time for others to book in and make use of the appointment slot. If you miss, reschedule, or cancel within 48 hours of your appointment time, you will be charged the regular fee for the appointment.

REBOOKING

Follow-up appointments are generally booked in person, and can also be easily booked online.

Cancellations, no-shows, and chronic rescheduling may require that you prepay for future appointments.

ARRIVING AT THE CLINIC

To accommodate everyone, I make every effort to adhere to our scheduled time slot. Please aim to arrive a few minutes early and make yourself comfortable in the waiting room. If you arrive late we will only have the remainder of your booked appointment time.

Please leave your shoes/boots in the waiting room, and remember to place your phone on silent. Please do not leave any valuables in the waiting room.

HEALTH & WELLNESS

If you have an appointment scheduled and you are feeling unwell, or are experiencing cold or flu-like symptoms, contact me as soon as possible so that we can reschedule your appointment.

HEALTH SCREENING

Health screening questionnaires will no longer be sent out in advance of your appointment.

Although this screening is no longer mandatory, please continue to self-monitor in advance of any upcoming appointment and reschedule if necessary.

MASKS

If you would like to, please feel free to wear a mask during treatment.

I am happy to wear a mask during your appointment on request.

HAND HYGIENE

Please use hand sanitizer when entering the clinic, before you exit, and throughout your appointment as needed. There is also a sink in the public washroom by the elevators if you prefer to wash your hands with soap.

ADDITIONAL ATTENDEES

Please attend your appointment alone. Exceptions include a caregiver, or a guardian for a minor’s appointment. Note that minors must be accompanied for the duration of their treatment.

If there is an additional person attending the appointment, please indicate this when booking.

A NOTE ABOUT COVID-19

Prior to the COVID-19 pandemic I was sanitizing surfaces and practicing hand hygiene between every appointment in my practice. I continue to do so, and additionally adhere to best practice guidance outlined by the Ministry of Health, Public Health, and the Ontario Osteopathic Association in response to the COVID-19 pandemic. If you have any questions or concerns about these practices please feel free to ask.
